Financial Policy

All co-pay/charges are due at time of service. 

Family Medicine/Chiropractic patients: If your insurance carrier does not pay or denies your claim, you will be responsible for the service. Patients who wish to see providers at Vitaus Medical outside their assigned insurance plan guidelines must pay for services and be reimbursed by the insurance company. PPO/HMO patients are expected to confirm with their insurance plans that Dr. Chris Eisenhauer is the participating provider. It is the patient's responsibility to inform the office of changes in policies, family member coverage, or benefits. Claims returned to our office due to incorrect information will be forwarded to the patient and balance payments will be expected. 

Missed Appointments

Please call 24 hours prior to missing your appointment. 

**Due to the increased amount of no-call-no-shows we now have to charge $25 for each appointment missed. 

Late Arrivals

Many days we are fully booked. Please call to let us know you are running late. We can only accept a lateness of 15 minutes after appointment time otherwise we will need to reschedule you. Please be considerate of our time and the time of our other patients.

Referrals/Medicine Refills

All referrals and medicine refills require a visit with the doctor before being filled. Please call to schedule an appointment.
