What is a family practice doctor? What does a family medicine doctor do?

A Family Medicine doctor specializes in providing care for all members of the family, and offers treatment for most medical problems. A Family Medicine Physician can provide all over care from acute/urgent illnesses and injuries to chronic disease states such as high blood pressure and diabetes to mental health issues such as depression and anxiety. Dr. Eisenhauer focuses on the “wellness” aspect of healthcare, drawing attention to preventative measures such as healthy habits with diet, exercise, and a well-rounded lifestyle. He can perform minor procedures in the office and has specialized training in surgical techniques.

Do I need to be seen by the doctor?

There are many situations that require a visit to the office. These may include:

​

  • Over 1 year since your last physical exam.

  • You believe you need a change of medication or strength of medication.

  • An illness or injury that is not well controlled using over the counter medications/therapeutics.

  • Patients needing signed documentation for a sports physical, life insurance, or accident. 

  • Some medications require close monitoring, and office visits will be done more frequently.

  • Second opinions or referrals to specialists.

  • A follow up to an inpatient hospital stay or surgery.

Do you offer same-day appointments?

Yes, we may offer same-day appointments, depending on the provider you would like to see.

What do we need to bring at the time of visit?

​

You need to carry your health insurance card and picture identity card. If your appointment concerns a medical report, bring it if you have it. New patients should bring all their prescription bottles along with names, addresses, and phone numbers of previous providers.

How can I get records from my previous doctor?

​

We will be happy to get your previous records from your primary care doctor or specialists you may have seen. Please remember to talk to our front desk and fill/sign and authorize your Medical Records Release Form.

What is the Patient Portal?

​

The patient portal allows you to view your medical records online from the comfort of your own home. Through the portal, you can view your lab results, request appointments, send a message directly to the doctor, request medication refills, receive appointment reminders, request referrals, and more. You can access the Patient Portal from any computer or smartphone with an internet connection.

How do I get access to the Patient Portal?

To gain access to our secure server on Patient Portal and become web-enabled, sign up by providing us with a personal (non-work) email address. You will then receive an email with your username/password and instructions as to how to log on. After you are provided login information, please try to log in within 48 hours, as the password will expire.

I had labs done but I cannot see them on my portal.

​

Our physician tries to review the labs on the same day. If the patient is not able to review their labs on the portal, it is most likely that not all of the lab results have returned, and that the physician is waiting until all labs are resulted before releasing it to the portal.

However, if there is any significant abnormality noted in the labs, the physician will always get in touch with you even if other labs are pending.

How do I get my prescriptions refilled?

To provide patients with excellent medical care, any patient who is on a long-term medication must see Dr. Eisenhauer every 6 months for a medication review. You will get enough refills to last you until your next visit. If having to do a month to month medication please contact your pharmacy and they will send us a refill request form.

What do I do if I get ill or injured when the office is closed?

​

If you have a life threatening medical emergency, CALL 911! Otherwise, you can call the office and follow the prompts to contact the after hours physician. There are also a number of urgent care facilities that have convenient after hours appointments.
